B: I'm five feet five inches.
A: And...uh...hair color?
B: Well, it's...gray, I guess.
A: Uh...Let's see. And the...the...color of your eyes?
B: Um...hazel. What...do you need to know any other information?
A: Uh... No... I think... uh...
B: Do you need to know where we live? Where I had...uh... the last license?
A: Let's...let's put your address down.
B: Well, it's... right now, it's a post office box. Post Office Box 115.

A: 這張申請書上…嗯…要填上您的身高。您的身高多少？
B: 我身高五呎五吋。
A: 然後…，頭髮顏色呢？
B: 這個嘛，我想是灰色的吧！
A: 啊，還有，我看看，還有您的…眼睛的顏色呢？
B: 嗯，淡褐色。您還需要…什麼其他資料嗎？
A: 啊…我想沒有…
B: 您需不需要知道我的地址？以及我…上回申請駕照的地方？
A: 讓我來…我來把您的地址記下來。
B: 好吧，目前…我的通訊地址是用郵政信箱，郵政信箱115號。

A：Personal Status 指的是你的婚姻狀況，有時表格上還會列出選項讓你勾選，例如single 代表單身；married 代表已婚；separated 代表分居；divorced 代表離婚；widowed 代表喪偶。
